Utah Medical Cannabis Card:

Individuals with one or more qualifying conditions are authorized under the Utah Medical Cannabis Act to receive a medical cannabis patient card to treat specific qualifying conditions. I am a licensed QMP in the state of Utah.  The qualifying conditions that are within my scope of practice to treat are Posttraumatic Stress Disorder (PTSD) and Autism Spectrum Disorders (ASD).  Treatment entails an initial evaluation, signing a release of information to the Utah Dept. of Health to allow me to inform them of condition being treated, relevant symptoms and treatment details, and a 6 month follow up, then follow up every 6-12 months on and on going basis or whenever changes to treatment need to be made.

Medical Cannabis UT I​D Registration: 

Initial Psychiatric Evaluation & Treatment Planning (50 minutes): $185

Mandatory 6-12 month follow up (20-30 minutes): $85

Insurance companies will not be billed for Medical Cannabis Services.

RESTRICTIONS- Cannabis cards may not be issued to Federal Employees or CDL drivers. I advise you to consult a Utah attorney first.
