Individual Therapy Session (60 minutes): $135

In Network Contracted Provider: 

Please inform us if you wish to utilize insurance. It is your responsibility to verify and understand your insurance coverage and obtain prior authorization if needed.  If I am contracted with your insurance company, a claim will be submitted to them once. If they deny the claim, you will be responsible for the cost of the session.

Our contracts with insurance companies require us to collect your copay at the time of service.  If you have an unmet deductible, $85.00 will be due at the time of service.  The remaining balance will be billed to you after the insurance has sent a response of coverage.  A statement will be available to you in your patient portal for your records  and any amount owing after your insurance company adjusts the claim will be automatically paid with a credit card kept on file.

If I am NOT contracted with your insurance, payment in full is due at the time of service. You will be charged the self-pay rates. We will provide you with a “Super Bill” that contains the information insurance companies require to process a claim. You may submit this directly to your insurance provider. In some cases, the claim may be submitted on your behalf or as a courtesy. Any insurance reimbursement ​will be paid directly to you by your insurance company.
